Experience
 

Clinical

I introduced open access endoscopy, which has been welcomed by local General Practitioners, shortly after my arrival at St Peters Hospital in March 1992 and formalised an open access endoscopy system in September 1993. I have seen a significant rise in the referral rate to the hospital with repatriation of a number of referral practices including significant Fund Holding GP workload. I currently receive an average of 2100 new GP referrals each year.

I have also introduced a number of non-endoscopic investigations including breath Hydrogen testing, 13C breath urea testing, and oesophageal function testing including oesophageal manometry with 24 hour pH testing, and solid state biliary manometry. In collaboration with the Nuclear Medicine Physician, I have introduced SeHCAT scanning to assess bile salt homeostasis and TcHMPAO white cell scanning.
 

Administration/Committees

I was responsible for implementing a computerised discharge summary system in the Medical CMU (Aries), and have since developed my own computerised system and PAS interface (PASINT) to facilitate both inpatient and outpatient secretarial work, as well as allowing acquisition of diagnostic and other data for audit, research and retrieval purposes.

I represent the Medical CMU on the Blood Transfusion and the Drugs and Therapeutics Committee. I represent the Hospital Consultants on the Information Strategy Group.
 

Teaching

I am responsible for training a (Calman) Specialist Registrar and the post is recognised by the Regional Monospeciality Subcommittee for Gastroenterology as suitable for training to a level of year 4/5 (in view of specialised ERCP training experience available). I teach on the SHO and HO protected teaching timetable and also on the MRCP teaching course at St Peters. I have elective medical students allocated to my firm for teaching for most of the year.
 
Research - MD thesis

I spent two years at the Northern General Hospital, Sheffield, supported by Sheffield Gastric Cancer Research fund and a Trent Regional Health Authority research grant performing the work for an MD thesis entitled "Lactic Dehydrogenase and beta-Glucuronidase in the human stomach, with special reference to gastric cancer" which was awarded by the University of London in August 1988.

This involved measurement of tissue enzymes from resected stomachs and grading histopathology and also testing 450 patients from a general endoscopy population in order to evaluate this technique as a screening test for gastric cancer and in order to identify a group at increased risk from gastric cancer. I also investigated vital dye staining of stomachs both in resected stomachs and at endoscopy in order to localise and quantify intestinal metaplasia.
